In Fall of 2025, Formed updated to require users to log in with a password, one time codes, or with Google verification. If you do not already have a password signed up or are signing up for the first time you may be sent a One Time Code to your email address so that you may log in initially. 

Once you log in for the first time with the one time code you will be prompted to create a password for yourself.  You can always select "Later" if you are opting to not set up a password, but you will be asked to enter a one time code each time if you do so.
