iPad + Safari App 

  1. Click the Settings App
  2. Locate the Safari App in the listed Apps
  3. Scroll to the bottom of the menu and select Advanced
  4. Select Website Data
  5. Then click Show All Sites
  6. Click the Edit link in the top right corner
  7. Locate the cookie associated with ooyala.com and click Delete
  8. Return to the Safari App and refresh the FORMED page
